Liability Waiver
Must be signed by a parent or guardian before your athlete's first practice. Download, sign, and bring it with you (or email it ahead of time).
Code of Conduct Agreement
Review Overtime's Code of Conduct and submit the athlete & parent agreement before your first practice.
Heads up: the Liability Waiver and the Code of Conduct agreement must both be completed before your athlete steps on the mat for their first practice. Bring the signed waiver with you, or email it to us in advance so everything's on file before you arrive.
